Preschool Pre-Registration

Pre-Registration for 4-Year-Olds

If your child will be 4 on or before August 1, 2024, and you would like to see if your child is eligible to attend the state-funded preschool program in Hopkins County Schools, please complete the online pre-registration form at the link below.

The completion of the pre-registration form will require you to have a Google account to upload forms OR you may choose to email the needed forms to preschool@hopkins.kyschools.us. You MUST upload or email proof of household income for your child's eligibility for the program to be determined. You may use W-2's, pay stubs, or tax returns as proof of income.

In addition to proof of income, you may upload or email any of the following preschool application components that you have access to for your child:

  • Certified Birth Certificate or Proof of Birth
  • Kentucky Immunization Certificate
  • Kentucky School Entrance Physical/Preventative Health Care Exam
  • Kentucky School Eye Exam
  • Proof of household income is required to determine eligibility for the preschool program for children who will be 4 years old by August 1, 2024.

4-Year-Old Registration Form

  • Pre-Registration for 3-Year-Olds

    If your child will be 3 on or before August 1, 2024, and you suspect your child may have an educational disability or a significant developmental delay, and you would like to have your child screened, please complete the online pre-registration form. We will be providing developmental screenings to 3-year-olds by appointment only. Please complete the online pre-registration form at the link below.
