  • Kindergarten Registration Requirements

    KRS 158.030---any child who is five years of age or who may become five years of age by August 1st may enter a public kindergarten; any child who is six years old or may become six years of age by August 1st shall attend public school.

    Kindergarten is compulsory.  All children must attend.

    A child/parent must provide the following:

    • A legal birth certificate or other reliable proof* of age and identification
    • Proof of residency
    • A Social Security card
    • An up-to-date immunization certificate
    • School entrance physical exam on a Kentucky form
    • Current eye exam by an optometrist/ophthalmologist
    • Dental screening or examination on a Kentucky form

    * Types of "other reliable proof" of a student's identity and age may include but not be limited to Social Security card; passport; military identification or immigration card; baptismal certificate; copy of the record of baptism – notarized or duly certified and which reflects the date of the student’s birth; recording of student’s name and birth in a family Bible or other religious text; notarized statement from the parents or another relative or guardian as to the date of the student’s birth; prior school record indicating the date of the student’s birth; driver’s license or learner’s permit; adoption record; any religious record authorized by a religious official; affidavit of identity and age; any government document or court record reflecting the date of the student’s birth; oral proof when the native language of a parent or guardian is not a written language.

  • Grades 1-12 Registration

    To enroll a child in Hopkins County Schools, the student's legal/permanent residence must be in Hopkins County. To find out which school your child should attend, call the Department of Pupil Personnel at 270-825-6000, go to the School Attendance Zones website, or contact your local school.

    Transfer Within Hopkins County Schools

    Parent or Guardian must register child at his/her new school. Student must live in the school district where they register.

    The receiving school will request records from previous school. A Release of Records must be signed at the school where your child is enrolling for us to request records from his/her previous school.

    Transfer from Out of District

    Please provide the following information:

    • Certified Birth Certificate (If an official certificate is not provided, the parent/guardian must present one within 10 days after the student enrolls or the Kentucky State Police will be notified.)
    • Kentucky Immunization Certificate
    • Social Security card
    • Copy of school records
    • Proof of residency
    • Documentation from a medical provider of medical condition
    • Copy of the current IEP or 504 Plan if your child currently receives Special Education Services

    In addition to the documents listed above, students from Out of State also must have the following completed:

    • Physical Examination
      702 KAR 1:160 requires each child first entering a Kentucky public school or entering Grade 6 to present a Kentucky Preventative Health Care Examination Form.  This exam may be completed up to one year prior to initial entry. 
    • Eye Examination within 30 days after they enroll
      All new students must have an eye exam performed by an optometrist or an ophthalmologist that is documented on a Kentucky Eye Examination form for school entry.  This evidence shall be submitted to the school no later than January 1 of the first year a child is enrolled in the public school system.

    Other Considerations for Parents:

    All transfers from out-of-state or non-public schools will be placed on hold until student records are reviewed. Therefore, providing records could expedite the process.

  • Proof of Residency

    The following items will be accepted as Proof of Residency (A Kentucky post office box address is not sufficient proof of physical address.)

    Section 1 - At least one of the following showing current Physical Address:  

    • County Property Tax Bill  
    • Mortgage Statement  
    • Lease/Rental Agreement with Landlord contact information 

    AND 

    Section 2 - At least one of the following showing current Physical Address issued within the last two months:  

    • Utility Bill showing service for Physical Address (electric, gas, cable/internet)  
    • Pay slip, salary statement, or benefits letter (with financial information marked through)  
    • Bank statement (with financial information marked through)  
    • Insurance statement (with financial information marked through) 

     

    Hopkins County Schools will require additional proof of physical address including home visits if necessary. Your application will NOT be processed and your student will NOT be given an enrollment for the upcoming school year until/unless you verify the address. NOTE: If legal custody of a child is split between two parents, in addition to the documents listed above, you must also attach a certified copy of the court order so primary residency can be established. You are responsible to immediately inform the school of any changes to the court order
